participant agreement, release of liability & media release.

This is what you accept at checkout. It's short, it's plain, and you should actually read it — especially section six, because the film is half of what you're signing up for.

In consideration of being permitted to participate in THE LEAGUE, an adult recreational basketball league operated by Fivemore LLC ("the League"), I, the undersigned participant, agree as follows:

1. assumption of risk.

I understand basketball is a physical, contact sport played outdoors on public courts, and that participation carries inherent risks including but not limited to: sprains, fractures, concussion, cardiac events, heat-related illness, contact with other players or surfaces, weather conditions, and court/surface conditions. I am voluntarily participating with full knowledge of these risks and I assume all risk of injury, illness, death, or property damage arising from my participation.

2. fitness to play.

I certify that I am at least 18 years old, physically fit to participate, and have no medical condition that would make participation unsafe. I am responsible for my own health decisions, including stopping play at any time.

3. release and waiver.

To the fullest extent permitted by law, I release, waive, and discharge the League, Fivemore LLC, its owners, organizers, volunteers, scorekeepers, sponsors, and agents, and the owner of any courts or grounds where League games and events are held, from any and all claims, demands, or causes of action arising out of or related to my participation, including those arising from ordinary negligence of the released parties.

4. indemnification.

I agree to indemnify and hold harmless the released parties from any claim brought by me or on my behalf arising from my participation.

5. medical treatment.

I authorize the League to arrange emergency medical treatment on my behalf if I am unable to consent, at my expense. The League does not provide medical coverage; any participant-accident policy is excess/secondary as written by the carrier.

6. media release.

THE ONE TO READLeague games and events are photographed and filmed — recaps, player cards, and league content are part of what the League is.

I understand League games and events are photographed and filmed. I grant the League and Fivemore a perpetual, royalty-free license to use my name, image, likeness, and voice captured at League games and events, in any media, for League operations, recaps, and Fivemore's marketing and social content, without compensation.

7. code of conduct.

I have read the League rules and code of conduct and agree to them, including the zero-tolerance and escalation provisions and the no-refund consequence of removal for conduct.

8. refunds.

No refunds except a League-cancelled season, refunded pro-rata. Removal for conduct is not refunded.

9. governing law.

North Carolina. If any provision is held unenforceable, the rest survive.

How this gets signed: accepting the terms checkbox at checkout constitutes your agreement to this document, and the acceptance timestamp is stored with your registration. Team registrations cover the captain at checkout; every rostered player signs before Week 1 (the roster form comes to the captain's inbox, with paper copies on site Week 1 as backup).
